Notes for John McDuffy Hawkins
The following is a synopsis of the Civil War Pension File of John M. Hawkins, Application #307039, Certificate #498194, National Archives Records Administration:

On August 23, 1879, John M. Hawkins, age 34, of Antwerp, applied for "Declaration for Original Invalid Pension" at the Court of Common Plees, Paulding Co, Ohio. He states that he entered service on the Dec 28, 1863, as a Private in Company C of the 68th Ohio Volunteer Infantry, commanded by Patrick H. Mooney, and was discharged at Louisville, Kentucky, by reason of close of the War. He is described as 5 feet, 7 inches in height, fair complexion, light hair, blue eyes. In the line of service at Benturifle (sp?), North Carolina, on March 19, 1865, he received a severe injury to his right shoulder by a tree falling on him in battle. The tree was cut down presumably by a shot. The injury was dressed by the Regimental Surgeon. His current occupation is that of a Laborer. Witnesses to the claim were B.L Wentworth, and Joseph M. Guston.

On February 25, 1881, the Adjutant General's Office of the War Department, Washington D.C, issues a claim that John M. Hawkins was enrolled on December 28, 1863, in Company C of the 68th Regiment of Ohio Volunteers; was present April 30 1865; mustered out with Company on July 10, 1865; "the records of this office furnish no evidence of alleged injury".]

On October 3, 1889, John filed a "General Affidavit", Claim #807039, in Paulding Co, Ohio, witnessed by Frank Long, Notary Public. It states that John "was first afflicted with chronic diarrhea, near Vicksburg, Mississippi about May 1864. After his Regiment returned from veteran furlough, in the spring of 1864, they went from Caro, Illinois, to some place on the Mississippi River, and was sent to Moun City, to the Hospital for treatment. From there he was sent to Jefferson Barracks, in the state of Missouri, where he was taken sick with a fever, where by improper treatment he became salivated,and by exposure, while sick, contracted disease of the lungs, and was confined to said Hospital at Jefferson Barracks with said disease for more than one month, and from where he was furloughed home on sick furlough, and remained at home for the period of about three months. When he returned to his Regiment for duty, he also at or about the same time contracted disease of the stomach, which said disease, to-wit, chronic diarrhea and the results thereof, disease of the lungs and stomach have been continuous during each year since the said year 1864, and that during each year since his discharge in the year 1866, he has been unable to perform manual labor for more than three fourths of the time, on account of said diseases, as aforesaid. That during each year since his discharge he has resided in Antwerp, Ohio".

On March 29, 1915, John M. Hawkins responded to a questionnaire from the Department of the Interior, Bureau of Pensions: born 1845, Franklin Co, Indiana; served in Company C, 68th Regiment Ohio Volunteer Infantry; residence, Antwerp, Ohio; wife's maiden name Susan Sines; married at Antwerp, Ohio, by George Woodcock; official record of marriage is at Paulding Co Court; wife has not been previously married; presently living with first and only wife; names and dates of birth of all children living or dead: Emma Hawkins born 1867, Robert Hawkins born 1869, Edwin Hawkins born 1871, John Lewis Hawkins born 1873, Mary Hawkins born 1875, Bernard and Berthy, twins, born 1877, Herbert Hawkins born 1880, Carrie Hawkins born 1883, Clida Hawkins born 1885.

On July 17, 1918, at age 73, John Hawkins filed a Declaration for Pension (Act of May 11, 1912), which repeated previously stated information, and was witnessed by Clyde H. Sunday and Dr. E.D. Murphy, both of Antwerp, Ohio, signed by C.L. Buiell, Notary Public, Paulding Co, Ohio.

On September 2, 1921, at age 76, John Hawkins filed a Declaration for Pension (Act of May 1, 1920), which again repeated previously stated information, and was witnessed by Tom K. Zuber and G.A. Wait, both of Antwerp, Ohio, signed by C.J. Harris, Notary Public, Paulding Co, Ohio.

On March 27, 1922, Susan Hawkins filed an Affidavit with the State of Ohio, Paulding Co, stating that "she is the wife of Hohn M. Hawkins, of Antwerp, Paulding Co, Ohio; that on account of her poor health, she was not able to take the care required, of John M. Hawkins, and that they did send for their son, Bernard Hawkins to come home and take care of them about July 15th, 1921. That she recognized the fact that her husband required better care than she could give him, was a worry that made it very burdensome for her. That about two years ago, his left arm was paralyzed and has regained but little use, if any use, of same since. That he has chronic Piles; that Rheumatism has bothered him for the past twenty-five years. That his lungs have given him a great amount of pain, and are very week; sensitive to the change of weather. That for the past eight months Bernard Hawkins has had the sole care of her husband, looking after his wants". Signed by C.A. Bissell, Notary Public.

On March 25, 1922, Bernard Hawkins filed an Affidavit with the State of Ohio, Paulding Co, stating that since the latter part of July, 1921, he has been employed by his father John M. Hawkins, to take care of him; that hes father does not require constant aid, but does require someone to be near, if he should require assistance. That his stomach has troubled him a great deal. Rheumatism is another complaint. Infirmities from old age, causing a general break down of his faculties. Senility is very noticeable. That he is not safe to be alone at anytime within the past year, and his mother looked after him as much as she could before he too sole charge of hem, commencing last July. That he does not require continuous aid, but requires someone on the watch all of the time. Signed by C.A. Bissell, Notary Public.

On October 9, 1922, John filed an affidavit stating that his son Bernie Hawkins, commenced to wait on him on July 5, 1921, and has been in constant care of him since said date, as his personal attendant". Signed by C.A. Bissell, Notary Public.

On October 9, 1922, Bernard Hawkins filed an affidavit stating that he had been caring for his father since July 5, 1921, that his father was unable to be left alone and required someone to be with him all of the time. Signed by C.A. Bissell, Notary Public.

On February 10, 1925, Susan Hawkins, age 74, filed for "Declaration for Widow's Pension" (Act of May 1, 1920), signed by C.A. Bissell, Notary Public, witnessed by Golda M. Starry and Bernard Hawkins, both of Antwerp, Ohio.

John M. Hawkins was last paid a pension of $72 per month on Jan 4, 1925.

John and his father Robert enlisted in the same Regiment/Company of the Ohio Volunteer Infantry.

John may have been at home on sick leave when his father was killed at the Battle of Atlanta. If not, then surely he was at his father's side when he fell in battle.
